About The Position

H2 Health is seeking a Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our outpatient team in Dublin, Virginia. Requirements

  • Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program (for PT role).
  • Current Virginia Physical Therapist license or eligibility for licensure (for PT role).
  • CPR certification or willingness to obtain as required.
  • Associate degree from a CAPTE-accredited Physical Therapist Assistant program (for PTA role).
  • Current Virginia Physical Therapist Assistant license or eligibility for licensure (for PTA role).

Responsibilities

  • Perform comprehensive patient evaluations and develop individualized plans of care.
  • Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
  • Treat a variety of orthopedic, post-operative, musculoskeletal, balance, vestibular, and mobility-related conditions.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
  • Educate patients on injury prevention, wellness, and home exercise programs.
  • Maintain accurate and timely documentation of patient progress and treatment outcomes.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
  • Provide skilled physical therapy treatment under the direction of a licensed Physical Therapist (for PTA role).
  • Implement individualized treatment plans to improve mobility, strength, balance, and functional independence (for PTA role).
  • Educate patients and caregivers on home exercise programs and injury prevention strategies (for PTA role).
  • Accurately document patient progress and communicate treatment updates with the clinical team (for PTA role).
  • Collaborate with Physical Therapists and support staff to support strong patient outcomes (for PTA role).
