About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Rolling Hills Estates and the surrounding areas. As an independent contractor, you will have the flexibility to control your own schedule, work in your preferred coverage areas, and determine your own earnings through strong per-visit rates. FeldCare Connects provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance, allowing you to focus on one-on-one patient treatment. The company utilizes an intuitive Electronic Medical Record system for easy documentation, often completable during visits. FeldCare Connects connects over 2,000 clinicians to patients in their communities by contracting with over 900 home health agencies throughout CA, AZ, TX, and FL, aiming to provide essential care to homebound patients and improve patient outcomes by reducing the administrative burden on agencies and clinicians.

Requirements

• Current Physical Therapist license issued by the state board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Ability and enthusiasm in working with all ages.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using an intuitive Electronic Medical Record system.
