About The Position
Physical Therapist (PT) / Occupational Therapist (OT) We are seeking dedicated and compassionate Physical Therapists and Occupational Therapists to join our team at Community Home Health Care in Fort Wayne, IN. Requirements
- Licensed Physical Therapist or Occupational Therapist in the state of Indiana.
- Strong clinical skills with a passion for home health care.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time effectively in a home care setting.
- Valid driver’s license and reliable transportation.
Responsibilities
- Conduct thorough patient evaluations to develop individualized treatment plans.
- Deliver personalized therapy sessions focusing on improving strength, coordination, and daily living skills.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families on therapeutic techniques and health maintenance.
- Maintain accurate and timely documentation in compliance with agency and regulatory standards.
