About The Position

This role is for a Physical Therapist in the Inpatient department at Ascension Alexian Brothers Rehabilitation Hospital in Elk Grove Village, IL. The position is PRN with a day shift schedule and a salary range of $53.00 - $56.00 per hour. The physical therapist will be responsible for developing and implementing patient-centered care plans, delivering evidence-based interventions for complex diagnoses, collaborating with an interdisciplinary team for care coordination and discharge planning, and utilizing rehabilitation technologies to maximize functional recovery.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Physical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.
Bachelor's degree required.
Prior Inpatient experience.

Nice To Haves

Master's OR Doctorate degree preferred.

Responsibilities

Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families.
Deliver evidence-based interventions across complex diagnoses, including neurological, oncological, orthopedic, cardiopulmonary, and medically complex conditions.
Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.
Utilize innovative rehabilitation technologies and best practices to maximize functional recovery and independence.
