About The Position

A leading healthcare provider is seeking a Physical Therapist, Inpatient Rehabilitation, to evaluate and treat patients of all ages in an inpatient rehabilitation setting, develop individualized treatment plans, deliver evidence-based therapy services, educate patients and caregivers, and collaborate with interdisciplinary teams.

Requirements

Bachelor's degree in Physical Therapy.
Current Illinois Physical Therapist (PT) License.
Basic Life Support (BLS) certification through the American Heart Association (AHA).
Knowledge of evidence-based rehabilitation practices, patient assessment techniques, treatment planning, and discharge planning.
Strong communication and interpersonal skills with the ability to educate patients, families, caregivers, and interdisciplinary team members.
Ability to safely perform patient transfers and mobility assistance while demonstrating proper body mechanics and patient safety practices.

Nice To Haves

Experience providing physical therapy services in an inpatient rehabilitation or acute care setting is preferred.

Responsibilities

Evaluate patients of all ages and develop individualized treatment plans that integrate functional goals, therapeutic interventions, expected outcomes, and discharge planning.
Deliver evidence-based physical therapy services, monitor patient progress, and modify treatment plans as appropriate based on patient response and clinical findings.
Perform safe patient transfers and mobility training with assistance as needed while utilizing proper body mechanics and promoting a safe environment for patients and staff.
Educate patients, family members, caregivers, students, and healthcare professionals using effective teaching methods to support recovery, independence, and continuity of care.
Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to coordinate patient care and facilitate successful rehabilitation outcomes.
Serve as a clinical resource and professional role model by demonstrating compassionate, respectful, and patient-centered care while supporting staff and student development.
