About The Position

Sutter Care at Home - Outer Bay is seeking a Physical Therapist III for home health services in Sonoma County. This role involves assessing, planning, organizing, and participating in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions. The Physical Therapist III will demonstrate advanced knowledge and practice level competencies, serving as a clinical or workflow subject matter expert. Responsibilities include developing key workflows, clinical protocols, and guidelines. This position also acts as a resource for education, training, and mentoring of other staff, and may serve as a clinical instructor for students. Adherence to all local, state, and federal regulations, codes, policies, and procedures is required to ensure privacy, safety, and optimal patient care.

Requirements

  • Graduate of an accredited physical therapy school
  • PT-Physical Therapist license
  • BLS-Basic Life Support Healthcare Provider certification
  • Valid Driver's License
  • Automobile Insurance
  • 5 years of recent relevant experience.
  • Knowledge and application of professional practice and regulatory requirements.
  • Ability to work in a demanding work environment, organize multiple priorities, and complete work duties within expected timelines and requirements.
  • Computer and required technology proficiency/competencies.
  • Effective and efficient professional communication (verbal & written) skills.
  • Interpersonal skills, conflict resolution, teamwork/collaboration, customer service, and community relations abilities.
  • Ability to retain and apply new knowledge & skills.
  • Ability to teach/educate others.
  • Ability to extract evidence-based practice information and apply to clinical practice and training materials.

Responsibilities
