This position will support four Atrium Health Inpatient facilities and three Acute Care facilities: Carolinas Rehabilitation Charlotte, Carolinas Rehabilitation Mount Holly, Carolinas Rehabilitation Northeast, Pineville Rehabilitation Hospital, Atrium Health Pineville, Atrium Health Mercy, and Atrium Health University. The Physical Therapist II is responsible for interpreting medical records and provider referrals, completing evaluations, and establishing patient care plans using evidence-based interventions. They will recommend the next level of care, supervise support staff, and ensure quality outcomes. Treatments may include education, exercise, therapeutic groups, modalities, gait, balance, coordination activities, sensory motor development, functional mobility, and transfer skills. The role also involves identifying and utilizing appropriate assistive devices and adaptive strategies, setting functional goals with patients/caregivers, monitoring outcomes, and collaborating with other disciplines for coordinated care. Effective communication with patients, caregivers, providers, and staff is essential, as is accurate and timely documentation for patient care, billing, and compliance. The position requires maintaining productivity standards, ensuring a clean and orderly work environment, and pursuing professional development. The incumbent will actively engage in performance improvement initiatives and manage their caseload, assisting with coverage needs across assigned departments and locations as part of the Enterprise Float Program. This requires adaptability to varying clinical and operational environments.
