Requirements
- Current Florida Physical Therapist (PT) license
- Valid driver’s license and reliable transportation
- Excellent communication and organizational skills
Nice To Haves
- Home health experience preferred, but not required
- Experience with WellSky or another home health EMR is a plus
Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized plans of care
- Evaluate and treat patients to improve mobility, strength, balance, endurance, and functional independence
- Develop evidence-based treatment plans and modify them based on patient progress
- Educate patients and caregivers on mobility, fall prevention, home exercise programs, and safe functional movement
- Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
- Complete accurate and timely documentation while maintaining Medicare and agency standards
