Day in the Life of a Physical Therapist with Wesleylife At Home: Empower clients through personalized care. Assess clients in their homes and develop individualized treatment plans that support mobility, safety, and functional goals. Deliver one‑on‑one therapy, using tailored exercises, manual interventions, gait training, and evidence‑based techniques. Educate & support clients and caregivers how to safely navigate daily activities, manage pain, and make simple home modifications that support independence. Collaborate & communicate with interdisciplinary team to ensure care is coordinated, holistic, and aligned with the client’s goals. Monitor progress and adjust plans as needs evolve, ensuring clients receive high‑quality, responsive support. Assess patients at home and create personalized treatment plans to improve mobility and function. Deliver tailored therapy including exercises, manual techniques, and gait training. Educate patients and caregivers on safe practices, pain management, and home modifications. Collaborate with healthcare teams to ensure coordinated and continuous care. Communicate progress and adjust plans based on patient needs and team input.
