About The Position

RVNAhealth is seeking compassionate, skilled Physical Therapists to join their award-winning Home Health team serving the Central Territory. This role offers greater autonomy, meaningful one-on-one patient care, and the opportunity to help patients recover and regain strength, confidence, and independence in their homes. The position supports therapists transitioning from other settings and provides a structured territory to minimize travel. Therapists will work as part of an experienced interdisciplinary team committed to exceptional patient care.

Requirements

Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate)
Current Connecticut Physical Therapist license
Reliable transportation and a valid driver's license

Nice To Haves

Minimum of two years of clinical experience preferred
Home health experience and OASIS documentation experience preferred, but not required
Excellent clinical assessment, communication, and time management skills
Ability to work independently while collaborating as part of a multidisciplinary team

Responsibilities

Performing comprehensive evaluations, reassessments, start-of-care visits, recertifications, and discharges
Developing individualized treatment plans focused on each patient's functional goals
Providing evidence-based interventions to improve strength, balance, mobility, endurance, and safety
Educating patients and caregivers on home exercise programs, fall prevention, disease management, and safe mobility
Collaborating closely with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary care team
Completing accurate, timely clinical documentation, including OASIS assessments
Monitoring patient progress and modifying treatment plans to achieve the best possible outcomes
