About The Position
Benchmark Therapy and Rehab is a large therapy practice providing services to a multitude of providers in Home Health, and we are seeking compassionate and driven Physical Therapists to join our therapy team. For this Opportunity: Benchmark Therapy provides therapy in geriatric healthcare in the home settings that produces measurable outcomes through therapy interventions to truly make a difference in someone's life. Requirements
- Requires state license to practice physical therapy.
Responsibilities
- Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.
- Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
- Works cooperatively with physicians, case managers and treating therapists.
- Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
