About The Position

The P.A.C.E. (Program of All-Inclusive Care for the Elderly) program offers a variety of services, many provided onsite at a PACE Center. PACE serves as an alternative to nursing home care, coordinating preventive, primary, acute, and long-term care services through an interdisciplinary care team (IDT) to enable older individuals to continue living in the community. Senior Care Partners PACE is seeking a Physical Therapist – Float to provide compassionate, participant-centered care across their Kalamazoo and Calhoun County locations. This role allows the therapist to use their clinical expertise in a dynamic setting, helping older adults maintain mobility, independence, safety, and quality of life both at PACE centers and in the community.

Requirements

Doctorate in Physical Therapy (DPT), unless grandfathered under applicable requirements.
Graduate of an approved physical therapy program.
Current, unencumbered Michigan Physical Therapist license.
At least 1 year of experience working with frail, elderly, or long-term care populations.
Strong communication, organization, problem-solving, and collaboration skills.
Working knowledge of Microsoft Office.
Current AHA CPR certification or willingness to obtain certification.

Responsibilities

Evaluate participants’ mobility, physical abilities, and rehabilitation needs.
Develop and implement individualized physical therapy plans of care.
Provide therapy through therapeutic exercise, modalities, manual therapy, and other evidence-based interventions.
Float between Kalamazoo and Calhoun counties based on participant and program needs.
Collaborate closely with the Interdisciplinary Team (IDT) to coordinate participant-centered care.
Assess and recommend adaptive and durable medical equipment and support equipment transitions.
Educate participants, families, and caregivers to promote safety and independence.
Support physical wellness programming and help develop innovative programs for participants.
Provide guidance and mentorship to PTAs, rehab tech PCAs, and PT students when applicable.
Support participants transitioning home from hospitals, skilled nursing, assisted living, and other care settings.
Participate in quality improvement initiatives and promote exceptional customer service.
