About The Position
Navitas Healthcare, LLC is seeking an experienced Home Health Physical Therapist to provide individualized therapy services to patients in their homes. The PT will evaluate patient needs, develop treatment plans, and help patients improve mobility, function, and quality of life.
Requirements
- Active Physical Therapist License
- BLS Certification required
- Minimum 1 year of Skilled Home Health experience
- OASIS experience required
- Reliable transportation required
- Strong communication and patient care skills
Nice To Haves
- Home Health experience with diverse patient populations
- Knowledge of rehabilitation and mobility improvement programs
- Experience working independently in community settings
Responsibilities
- Perform physical therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Improve patient mobility, strength, balance, and functional independence
- Educate patients and caregivers on exercises and safety techniques
- Complete OASIS assessments and documentation
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with physicians and interdisciplinary healthcare teams
- Maintain accurate patient records using EPIC and OASIS systems
- Travel to patient homes within the assigned coverage area
