About The Position

This is a full-time (1.0 FTE) position for a Physical Therapist Case Manager in Home Health. The role involves working collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers, and community agencies to coordinate and oversee the interdisciplinary plan of care for patients in a home care environment. The position requires assessing, planning, facilitating, and evaluating timely coordination of quality, cost-effective care, with flexible hours and 24-hour accountability as necessary to meet patient/family care needs.

Requirements

• Bachelor's Degree in Physical Therapy or higher.
• State of WI Physical Therapist licensure.
• Valid State of WI Driver's License with acceptable driving record.
• Current BLS (CPR) certification or ability to obtain upon hire.

Nice To Haves

• At least 6 months experience in a home health setting preferred.
• At least 1 year physical therapy experience preferred.
