About The Position

We are seeking a Part Time Physical Therapist Case Manager for our Home Health division to cover Contra Costa County, specifically Richmond. In this role, you will work with other clinicians to provide skilled services in patients' homes, helping them live safely and productively. Your primary goals will be to restore function, minimize disability, and help clients reach their maximum potential. You will evaluate and treat patients using prescribed physical therapy programs and may supervise Physical Therapy Assistants (PTAs). All opportunities at 21HHS require visiting patients in their homes.

Requirements

  • Current unrestricted CA Physical Therapist License
  • Current BLS/CPR Certification
  • Valid CA Driver's License
  • Knowledge of local community resources
  • Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.

Responsibilities

  • Perform an evaluation in the home and develop a care plan.
  • Work with PTAs to follow the care plan.
  • Help patients with mobility, movement, and regaining independence.
  • Create detailed treatment and care plans for each patient.
  • Communicate and coordinate with patients’ physicians to ensure quality care.
  • Evaluate and report patients’ progress to MD.
