About The Position

The Physical Therapist Care Navigator (PTCN) is Motion MSK’s licensed clinical expert who sits at the center of the patient’s musculoskeletal care journey, helping patients navigate the often complex world of MSK care. In this role, you will apply your clinical expertise to triage patients, guide shared decision making conversations, and ensure they access the right care at the right time. As the clinical escalation point across the entire care journey, you will support both patients and providers in making confident, informed decisions about care. Through high-touch telephonic engagement, evidence-based education, and ongoing clinical guidance, you will build trusted relationships with patients while ensuring they receive coordinated, effective care across the healthcare ecosystem. This is a technology-enabled clinical navigation role designed to elevate the patient experience beyond traditional episodic care and help patients successfully move through their recovery journey.

Requirements

  • Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT)
  • Active, unrestricted Physical Therapist license in Oregon, with the ability to obtain and maintain licensure in additional states and/or PT Compact privileges as needed.
  • 5+ years of orthopedic or musculoskeletal clinical experience.
  • Strong clinical judgment and triage skills, with the ability to assess complex patient situations and guide appropriate care pathways.
  • Demonstrated experience in motivational interviewing, patient coaching, and supporting behavior change.
  • Comfortable working in a highly telephonic environment, with the majority of the workday spent supporting patients through inbound and outbound interactions.
  • Excellent interpersonal and communication skills, with the ability to build trust and engage patients from diverse backgrounds.
  • Experience collaborating within multidisciplinary care teams to support coordinated patient care.
  • Ability to work within structured clinical workflows and guardrails while maintaining strong patient rapport.
  • Strong comfort with technology, including EMR systems and remote clinical or care navigation tools.
  • Access to a private, secure, HIPAA-compliant remote work environment suitable for conducting confidential patient communications.

  • Experience delivering care or patient support in a telephonic, remote, or telemedicine environment, including the use of inbound and outbound call systems for care navigation or coordination.
  • Self-starter with demonstrated ability to adapt in a fast-paced, evolving healthcare environment.

Responsibilities

  • Serve as the clinical escalation point across the MSK care journey, guiding triage recommendations and resolving complex or unclear clinical situations.
  • Conduct evidence-based clinical intake assessments and use your clinical expertise to determine the most appropriate care pathway for patients.
  • Provide ongoing clinical guidance, education, and shared decision-making support, helping patients navigate treatment options and progress successfully through different stages of care.
  • Build trusted, empathetic relationships with patients through high-touch telephonic engagement, educating them on Motion MSK programs and available care resources.
  • Conduct outreach to eligible patients and provide proactive follow-up after clinical decisions to initiate or adjust care pathways and maintain continuity of care.
  • Monitor patient progress and re-triage care plans when necessary to ensure patients are progressing appropriately through their treatment pathways.
  • Provide ongoing support using motivational interviewing techniques to promote engagement, adherence, and persistence in care plans.
  • Support coordination between patients, caregivers, primary care providers, specialists, and affiliate providers.
  • Oversee navigation pathways for patients escalating to surgical care, ensuring clinical coordination between patient, PCP, and surgeon.
  • Collaborate closely with Care Coordinators, Outreach Specialists and other cross-functional partners.
  • Manage a high-volume phone workflow, supporting primarily inbound patient calls while conducting outbound outreach and follow-up as part of structured care navigation processes.
  • Document all patient interactions and care navigation activities within CRM and EMR systems to ensure accuracy, continuity, and visibility of care.
  • Manage structured workflows that support care coordination, clinical oversight, and a high-quality patient experience.
  • Perform other related duties as needed to support evolving patient care needs, care navigation workflows, and business priorities.
