Our care management program has been in existence for over 25 years and was originally part of the Lee Memorial Health System.

Requirements

Registered associate degree from a recognized Physical Therapy Assistant Program accredited by the American Physical Therapy Association.
Valid, current Florida license.

Responsibilities

Performs Services planned, delegated and supervised by the therapist
Assists in preparing clinical notes and progress reports
Participates in educating the patient and family
Provides services planned, delegated and supervised by the therapist
Participates in in-service programs
Must be able to communicate instructions to the patients
Must be able to establish and maintain effective working relationships with patients and home care staff
Participates in patient case conferences as requested
Maintains confidentiality of all patient, Agency and Employee matters
Attains quarterly goals as established in the annual performance evaluation
Submits required documentation in a timely manner.
