Requirements

  • Associate's Degree
  • BLS - Basic Life Support - American Heart Association
  • DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles
  • LIC - Licensed in Field - State of Ohio
  • PTA - Physical Therapist Assistant - American Board of Physical Therapy Specialties
  • Knowledge of home health Medicare regulations, JCAHO standards, home care reimbursement
  • Experience in an acute, rehabilitation or home health setting
  • Basic personal computer skills
  • Knowledge in assessment, planning, implementation, and evaluation of occupational therapy
  • BLS certification

Responsibilities

  • Driving is an essential function of the job.
  • Administers portions of the assessment that is assigned by the Physical Therapist (PT) in which Physical Therapy Assistant (PTA) has demonstrated to be competent.
  • Assists in reassessment process, reporting patient performance to the PT.
  • Assists PT in implementation of the patients plan of care based on occupational performance frames of reference(s): bio-mechanical, rehabilitation, sensorimotor.
  • Attends multidisciplinary patient care meetings.
  • Plans and helps coordinate discharge plan with multidisciplinary team.
  • Educates patient and caregiver/family.
  • Recommends Medical Equipment required for discharge.
  • Provides information to Physicians, Nurses, Care Managers, Social Workers, and other team members.
  • Documents evaluation performance directed and interpreted by the PT, progress towards goals, discharge plan/continuity of care for the multidisciplinary team, and other involved medical and insurance parties.
  • All documentation to be co-signed by the supervising PT.
  • Delegates assignments to Rehab Technicians.
  • Remains involved in department process improvements utilization review and quality assurance program.
  • Educates others about Physical Therapy.
  • Takes active role in orienting new staff members.
  • Assists in clinical instruction of PTA students.
  • Educates other disciplines and medical/hospital personnel on PT purpose, goals and treatment policies/procedures.
  • Assumes other duties delegated by management.
  • Focus on safety, quality and efficiency in performing job duties.
