About The Position

San Luis Care Center, a Life Care Center of America facility, is seeking a Licensed Physical Therapist Assistant (PTA) to provide direct rehabilitation care in physical therapy to patients under the supervision of a licensed Physical Therapist (PT). This role is crucial in implementing physical therapy treatment plans and ensuring patients receive high-quality care. The facility is located in Alamosa, Colorado, surrounded by the Colorado Mountains and near attractions like The Great Sand Dunes National Park. Requirements

  • Graduate of an accredited program in physical therapy (ASPT).
  • Currently registered/licensed in applicable State.
  • Must maintain an active license in good standing throughout employment.
  • CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
  • Must be proficient in Microsoft Word, Excel, and e-mail.
  • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
  • Read, write, speak, and understand the English language.

Responsibilities

  • Provides direct rehab care in physical therapy to patients upon physician referral under the supervision of a licensed Physical Therapist (PT).
  • Follows physical therapy treatment plans for patients under the direction of the supervising PT.
  • Charts appropriately and timely.
  • Utilizes therapy software appropriately and accurately.
  • Exhibits excellent customer service and a positive attitude towards patients.
  • Assists in the evacuation of patients.
  • Demonstrates dependable, regular attendance.
  • Concentrates and uses reasoning skills and good judgment.
  • Communicates and functions productively on an interdisciplinary team.
  • Participates in community health matters/projects as appropriate.
  • Liaisons with patients, families, support departments, etc., to adequately plan for patient needs.
  • Demonstrates good body mechanics at all times.
  • Makes independent decisions when circumstances warrant such action.
  • Is knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post-acute care facility.
  • Implements and interprets the programs, goals, objectives, policies, and procedures of the department.
  • Performs proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation.
  • Maintains professional working relationships with all associates, vendors, etc.
  • Maintains confidentiality of all proprietary and/or confidential information.
  • Understands and follows company policies including harassment and compliance procedures.
  • Displays integrity and professionalism by adhering to Life Care’s Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training.
