About The Position

Sequoia Transitional Care is looking for a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team. The Physical Therapist Assistant (PTA) provides skilled therapy services under the direction and supervision of a licensed Physical Therapist. The PTA assists in implementing individualized treatment plans, monitors resident progress, documents care, and collaborates with the interdisciplinary team to help residents achieve their highest level of function and independence.

Requirements

  • Associate Degree from an accredited Physical Therapist Assistant program
  • Current California Physical Therapist Assistant license in good standing
  • Current CPR certification if required by state licensure
  • Must maintain all continuing education requirements and state licensure
  • Excellent communication and interpersonal skills
  • Strong organizational and time management abilities
  • Ability to work collaboratively within an interdisciplinary healthcare team
  • Knowledge of rehabilitation techniques, therapeutic exercises, and resident safety practices
  • Commitment to delivering compassionate, resident-centered care
  • Frequent standing, walking, bending, reaching, lifting, pushing, and pulling
  • Ability to assist with resident transfers and mobility activities using safe patient handling techniques
  • Ability to exert significant physical effort while assisting residents and moving equipment
  • Ability to communicate effectively with residents, families, and healthcare professionals
  • Visual, auditory, and tactile abilities necessary to safely perform patient care duties

Nice To Haves

  • Previous experience in a skilled nursing or rehabilitation setting preferred but not required
  • Experience with Net Health and PointClickCare preferred
  • Strong computer and documentation skills

Responsibilities

  • Provide physical therapy treatments as directed by the supervising Physical Therapist
  • Complete timely, accurate, and compliant documentation in accordance with facility policies, state regulations, and third-party payer requirements
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols
  • Participate in Resident Care Conferences and Rehabilitation meetings as needed
  • Comply with all applicable California Physical Therapy Practice Act requirements
  • Assist nursing staff with training and education of Restorative Nursing Aides
  • Communicate regularly with the supervising Physical Therapist regarding resident progress, treatment goals, physician updates, and discharge planning
  • Educate residents, families, and caregivers on home exercise programs, safety techniques, fall prevention, and maintenance programs
  • Recommend and assist with obtaining durable medical equipment to maximize resident independence
  • Report equipment concerns promptly to ensure safe operation
  • Participate in quality assurance and performance improvement initiatives
  • Maintain confidentiality of resident and facility information
  • Foster positive working relationships with residents, families, physicians, and interdisciplinary team members
  • Perform other duties as assigned
