About The Position

HealthPro Heritage has a great opportunity for a Licensed Physical Therapist Assistant (LPTA) at National Church First Community Village in Columbus, OH. This is a full-time, outpatient role within a senior living community, focusing on treating motivated residents in a functional environment aimed at independence and quality of life. The position offers a consistent caseload, continuity of care, and the chance to build meaningful relationships with patients. You will work alongside experienced PTs, OTs, and SLPs in a supportive, team-oriented environment with access to a modern outpatient gym and equipment. The role emphasizes functional, patient-centered care, focusing on mobility, balance, fall prevention, and maintaining independence. The predictable schedule offers a good work-life balance with typically fewer evenings and weekends, and no on-call demands. There are opportunities for mentorship, training, and continuing education (CEUs), and the work is set on a beautiful campus within a mission-driven organization (National Church Residences) focused on improving the lives of older adults. There is also potential to expand skills across outpatient and other levels of care within the community.

Requirements

  • Associate’s degree in Physical Therapist Assisting from an accredited program.
  • Valid state licensure as a Physical Therapist Assistant, or license eligible
  • Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
  • Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

  • Assist in implementing treatment plans designed by Physical Therapists.
  • Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
  • Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
  • Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
  • Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
