About The Position

Stern at Home Therapy is seeking a Full-Time or Part-Time Physical Therapist Assistant (PTA) to deliver one-on-one physical therapy treatments at an established Assisted Living Facility (ALF) in Port Jervis, NY. This role offers the stability of a facility-based caseload without the travel demands of traditional home health, allowing therapists to focus on patient care at a single location. The PTA will operate under the guidance of licensed Physical Therapists who are responsible for establishing patient care plans and providing ongoing clinical supervision.

Requirements

Active Physical Therapist Assistant license in New York required.
Graduate of an accredited Physical Therapist Assistant program.
Strong clinical, communication, documentation, and time-management skills.
Ability to work independently while collaborating with supervising Physical Therapists and facility staff.

Nice To Haves

Geriatric, assisted living, skilled nursing, outpatient, or home health experience preferred but not required.

Responsibilities

Provide one-on-one physical therapy treatments at a single assisted living facility.
Carry out individualized plans of care established by the supervising Physical Therapist.
Help patients improve strength, balance, mobility, endurance, safety, and overall functional independence.
Provide therapeutic exercise, gait training, transfer training, balance activities, and other interventions outlined in the plan of care.
Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
Educate patients and caregivers on exercises, mobility strategies, and safety.
Complete accurate and timely clinical documentation.
Communicate with patients, families, facility staff, supervising PTs, and interdisciplinary care team members.
Coordinate with Stern's administrative team regarding scheduling and patient care.
