H2 Health is seeking a Physical Therapist Assistant (PTA) to join our outpatient rehabilitation team in Eddyville, Kentucky. Requirements

  • Associate degree from a CAPTE-accredited Physical Therapist Assistant program.
  • Current Kentucky Physical Therapist Assistant license or eligibility for licensure.
  • CPR certification or willingness to obtain as required.
  • New graduates are encouraged to apply.

Responsibilities

  • Deliver skilled physical therapy treatments under the direction and supervision of a Physical Therapist.
  • Assist patients in achieving functional goals through evidence-based therapeutic interventions.
  • Provide patient and caregiver education to support continued progress outside of therapy sessions.
  • Monitor patient response to treatment and communicate progress with the supervising Physical Therapist.
  • Maintain accurate and timely documentation of patient care and treatment progress.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Manage a typical full-time outpatient caseload averaging approximately 10-12 patient visits per day.
  • Collaborate with Physical Therapists and other healthcare professionals to support individualized plans of care.
  • Contribute to a positive, patient-first clinical environment focused on exceptional outcomes.
