About The Position

HealthPro Heritage has a great opportunity for a Licensed Physical Therapist Assistant (LPTA) at The Heritage at Brentwood in Brentwood, TN. This full-time position is located in a beautiful upscale senior living community on a scenic 48-acre campus with Assisted Living, Skilled Nursing, Rehabilitation, and Memory Care all in one location. The role offers a variety of clinical experience by treating short-term rehabilitation patients alongside long-term residents, helping you grow your orthopedic, neurological, balance, and mobility skills. You will work within a collaborative interdisciplinary team with Physical Therapists, Occupational Therapists, Speech-Language Pathologists, nursing staff, and physicians, all working together to achieve excellent patient outcomes. Requirements

  • Associate’s degree in Physical Therapist Assisting from an accredited program.
  • Valid state licensure as a Physical Therapist Assistant, or license eligible.
  • Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
  • Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

  • Assist in implementing treatment plans designed by Physical Therapists.
  • Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
  • Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
  • Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
  • Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
