About The Position

Exceptional Rehab Services, located within the Charlestown Place facility, is dedicated to delivering high-quality, compassionate therapy services to individuals in need of rehabilitative care. We focus on improving the quality of life for our residents through personalized, attentive care and fostering a supportive, team-oriented environment. We are seeking a motivated and compassionate Full Time or PRN Physical Therapist Assistant (PTA) to join our team. The ideal candidate will be passionate about helping residents achieve their fullest potential while collaborating with a skilled team of therapists and healthcare professionals.

Requirements

  • Licensed PTA in the state the facility is located in
  • Strong communication and teamwork skills

Nice To Haves

  • Prior long-term care and EMR experience strongly preferred
  • Experience in SNF setting is a plus but not required.
  • A passion for helping others thrive!

Responsibilities

  • Work with a diverse range of rehab patients and rehab needs
  • Collaborate with a fun and supportive interdisciplinary team
  • Deliver personalized treatment plans in a welcoming, community-centered environment
  • Provides therapy and performs selected physical therapy interventions under the direction and supervision of the Physical Therapist.
  • Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department
  • Obtains data related to the interventions provided.
  • Makes modifications in selected interventions either to progress the resident as directed by the physical therapist or to ensure resident safety and comfort.
  • Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities.
  • Carries out the d/c plan as determined by the PT
  • Maintains proper documentation
  • This includes but is not limited to: creating and maintaining a resident record for each patient treated; completing documentation per policy including timeliness and medical necessity support; assures therapy billing and labor entries are complete, accurate and timely
  • Communicates effectively by maintaining communication with supervisory PT; participating in supervisory visits with the supervising PT as required by the State Practice Act; requesting reexaminations from the supervising PT when a change in resident status is identified; partnering with multi-disciplinary team to carry out plan of care and advocate for resident and communicating with the interdisciplinary team regarding treatments and discharge planning.
  • Relates professionally, positively, and cooperatively with residents, family members, and other employees at all levels
