Requirements

  • Must have a current PTA license with the Minnesota Board of Physical Therapy and either have or be open to obtaining a WI license.
  • Must have an Associate's degree in Physical Therapy Assisting.
  • At least one year of previous experience as a PTA in home care settings.
  • Must be comfortable/familiar with Medicare visits and have knowledge of OASIS Start of Care process.
  • Must demonstrate knowledge of age related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co-morbidities affecting care.
  • Proficiency with communication and data processing equipment, clinical apparatus related to home care, electronic medical record system, OASIS evaluation and data set.
  • Must have reliable transportation for client visits.

Responsibilities

  • Assess physical, psychological, social and environmental needs of the patient and family.
  • Maintain necessary records and use them appropriately for the care of patients, and demonstrate proficiency in the OASIS data set time points.
  • Provide patient/family education including patients self care and care provided by family.
  • Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.
  • Assess needs and refer to Adult Protection Services regarding status of vulnerable adults as necessary.
  • Provide skilled clinical care including evaluation and treatment of patients, and the implementation of care plans.
  • Anticipate the needs of the population served as well as the unique characteristics of the communities served.
  • All other duties as assigned.
