About The Position

Signature Healthcare at Home is seeking a Full Time Physical Therapist Assistant (PTA) to join their home health team, serving the East Side of Portland, OR. This role involves providing one-on-one patient care in their homes under the supervision of a licensed Physical Therapist. Requirements

Licensed Physical Therapist Assistant (PTA) in Oregon
Graduate of an accredited PTA program
Current driver’s license and reliable transportation
Ability to work independently in a field-based setting

Nice To Haves

Home health experience
Familiarity with OASIS and Medicare documentation
Strong time management and clinical skills

Responsibilities

Provide physical therapy treatments according to the plan of care established by the supervising PT.
Assist patients in improving mobility, strength, balance, and functional independence.
Observe and report patient responses to treatment.
Work closely with Physical Therapists, nurses, and the interdisciplinary care team.
Communicate patient progress and recommend adjustments to care plans.
Participate in care coordination and case discussions.
Instruct patients and families on home exercise programs, safety and fall prevention strategies, and proper use of assistive devices.
Complete accurate, timely clinical documentation.
Follow Medicare home health guidelines and Oregon state regulations.
Ensure compliance with company and regulatory standards.
Support patient-centered outcomes and satisfaction.
Help reduce hospital readmissions and promote independence.
