The Physical Therapist Assistant (PTA) will manage an assigned patient caseload daily, ensuring timely care and documentation per the plan of care and physician orders. This role involves collecting health data, implementing care programs, documenting and executing care plans, and providing patient/family education. The PTA will apply clinical reasoning for assessments, therapy effectiveness, and prioritization of needs. They will observe, record, and report treatment reactions or condition changes to the physician/PT, and communicate updates with the multidisciplinary team. The position requires incorporating short- and long-term goals into therapy and ensuring clear, timely, and complete documentation to meet productivity targets. The PTA will perform treatments per State PT Practice Act, Home Health Conditions, JCAHO, Medicare, OASIS, and agency policy, while also promoting shared governance and integrating evidence-based practice. Maintaining current PT knowledge and competence, collaborating with patients and families, and evaluating their own practice are also key aspects of this role. The PTA will utilize appropriate resources for safe, effective, and cost-conscious PT services.
