Requirements
- Evaluate patients and create individualized plans of care
- Deliver hands-on, patient centered treatment and education
- Monitor progress and adjust care plans to achieve meaningful outcomes
Responsibilities
- Assist in evaluation of client’s daily living skills and capacities to determine extent of abilities and limitations
- Assist patients with all phases of physical therapy treatment and techniques including exercise instruction, manual therapy techniques, activities of daily living
- Observe patients during treatments and reports observations to Physical Therapist
- Communicate with staff and other health team members, to exchange, discuss, and evaluate patient information for planning, modifying, and coordinating treatment programs
