About The Position
Home Health Care, Inc. is seeking a Licensed Physical Therapist Assistant (PTA) to join their team. This role focuses on helping patients achieve their personal goals, whether it's walking safely, returning to hobbies, or simply navigating their home environment without fear. As a PTA in home health, you will provide one-on-one patient care in the patient's home, fostering functional therapy and building meaningful relationships with patients and their families. The position offers greater flexibility and independence compared to traditional settings, with a supportive team of experienced professionals.
Requirements
- Active Minnesota Licensed Physical Therapist Assistant (PTA) license.
- Compassionate, patient-centered approach to care.
- Strong communication and organizational skills.
- Valid driver's license and reliable transportation.
Nice To Haves
- Home health experience is helpful but not required.
Responsibilities
- Deliver therapeutic interventions established by the supervising Physical Therapist.
- Assist patients with strengthening, balance, gait training, transfers, and mobility exercises.
- Reinforce home exercise programs and educate caregivers.
- Monitor patient progress and communicate updates to the supervising PT.
- Collaborate with physicians and the interdisciplinary care team.
- Complete accurate, timely documentation.
