About The Position

Transitions At Home is seeking to hire a Full-Time Licensed Physical Therapist Assistant in the Rice Lake area. This role involves administering physical therapy to patients on an intermittent basis in their place of residence, in accordance with physician orders and the plan of care, under the direction and supervision of the Physical Therapist/RN Clinical Manager. The ideal candidate is caring, compassionate, and appreciates a flexible work schedule, independence, and a work family team culture.

Requirements

• Active Wisconsin Physical Therapist Assistant License
• Valid CPR Certification
• Valid Wisconsin Driver's License and Car Insurance

Nice To Haves

• Two (2) years’ experience preferred.

Responsibilities

• Administer physical therapy to patients on an intermittent basis in their place of residence.
• Follow physician orders and plan of care.
• Work under the direction and supervision of the Physical Therapist/RN Clinical Manager.
