About the Role: Patient Care & Rehabilitation Assist the licensed Physical Therapist in implementing individualized treatment plans to support patient goals and functional outcomes. Provide therapeutic interventions, including prescribed exercises, therapeutic activities, manual techniques, and patient education, within the scope of practice and organizational policies. Monitor patients throughout treatment and communicate changes in condition, progress, or concerns to the supervising Physical Therapist. Educate patients and families on prescribed exercises, treatment plans, and strategies to support continued progress and overall well-being. Encourage and motivate patients throughout their rehabilitation journey, creating a positive and supportive therapy experience. Maintain accurate, timely, and comprehensive documentation in accordance with legal, regulatory, and professional standards. Collaboration & Team Support Collaborate with Physical Therapists and other healthcare professionals to promote coordinated, comprehensive patient care. Communicate effectively with clinical and administrative team members to support continuity of care and an exceptional patient experience. Participate in team meetings, staff training, continuing education, and quality improvement initiatives as required. Follow established safety, infection control, compliance, and organizational policies and procedures. Contribute to a positive team environment focused on patient outcomes, service excellence, and continuous improvement. This role will be based at Evara Health’s new Physical and Occupational Therapy Center, located at our headquarters at 14100 58th Street N. in Clearwater. This newly developed space is designed to expand access to rehabilitation services for the patients and communities we serve, giving you the opportunity to be part of a growing program from the ground up. Requirements

  • Physical Therapist Assistant degree from an accredited program
  • Valid and active state licensure as a Physical Therapist Assistant
