About The Position
We are seeking a compassionate PTA to provide skilled, patient-centered care under the direction of a Physical Therapist. Provide skilled PTA services in patients’ homes, assist with treatment plans, monitor progress, educate patients and families, and ensure accurate documentation in compliance with agency and Medicare guidelines. Work closely with Physical Therapists and care teams to support improved patient outcomes.
Requirements
- Active PTA license (state of employment)
- 1+ year acute care PTA experience required
- Valid driver’s license + auto insurance
- Strong communication, documentation, and clinical skills
Nice To Haves
- Home health experience preferred
