About The Position

Heralds Home Health is seeking an experienced, compassionate, and clinically skilled Physical Therapist Assistant (PTA) to join its Home Health team serving patients throughout the South Bay area of Los Angeles County. The role provides personalized, one-on-one care to help patients regain strength, mobility, and independence in their homes. The position is per diem/part-time, with flexible scheduling opportunities and competitive compensation based on experience and visit type.

Requirements

Active California Physical Therapist Assistant (PTA) license.
Strong clinical skills and knowledge of rehabilitation techniques.
Ability to manage field visits efficiently and maintain accurate documentation.
Strong communication and patient education skills.
Reliable transportation and willingness to travel throughout the South Bay area.

Nice To Haves

Home Health experience preferred.
Experience working with geriatric patients and individuals with mobility limitations preferred.
Bilingual Spanish or Vietnamese preferred.

Responsibilities

Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
Implement established treatment plans to improve patient mobility, strength, balance, and functional independence.
Assist patients with gait training, therapeutic exercises, transfer training, and fall prevention.
Monitor patient progress and promptly communicate changes in condition to the supervising Physical Therapist.
Educate patients and caregivers on prescribed exercises, mobility techniques, and safety precautions.
Complete accurate and timely clinical documentation.
Collaborate with the interdisciplinary healthcare team to support positive patient outcomes.
