H2 Health Prime Living is seeking a Physical Therapist Assistant to provide 1:1 patient care within luxury Senior Living communities in Paducah. Requirements
- Graduate of an accredited Physical Therapist Assistant program.
- Active or eligible state license to practice.

Responsibilities
- Provide 1:1 patient care within Senior Living communities.
- Collaborate with an interdisciplinary team to provide therapy opportunities.
- Act as a provider and an extension of the communities served.
