About The Position
Synergy Care is looking for a qualified Licensed Physical Therapist Assistant for an open Part Time position in Normal, IL. We are looking for someone who will provide quality patient care and be a team player.
Requirements
- Licensed Physical Therapist Assistant
- Graduate of an accredited school for Physical Therapist Assistant or will graduate from such an institution within the next six (6) months.

Responsibilities
- Provides physical therapy and related services for patients, caregivers and facilities in accordance with the principles and practices of occupational therapy and with Synergy Management Solutions guidelines.
- Provides quality PT including: Evaluation of patients, development of initial and ongoing treatment & discharge plan, supervision of assistants and aides & participates in case management.
- Represents Synergy Management Solutions’ commitment to quality & service in all interactions with facility.
- Adheres to all state regulations regarding occupational therapy.
