About The Position

Concept Rehab is a therapist-owned and therapist-led rehabilitation company with over 45 years of experience providing exceptional rehabilitation services in Skilled Nursing Facilities and post-acute care settings. We are seeking a passionate Physical Therapist Assistant (PTA) to join our team and help patients regain mobility, independence, and quality of life. We believe in supporting our clinicians with experienced therapy leaders who understand patient care.

Requirements

Graduate of an accredited Physical Therapist Assistant program
Associate degree as a Physical Therapist Assistant
Current PTA license in the state of practice
Strong communication, teamwork, and clinical problem-solving skills
Passion for delivering exceptional patient-centered care in a Skilled Nursing Facility setting

Responsibilities

Deliver physical therapy treatments according to established plans of care under the supervision of a Licensed Physical Therapist.
Help patients improve mobility, strength, balance, function, and independence.
Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team.
Collaborate with physicians, nursing staff, caregivers, and families to support patient outcomes.
Participate in care planning and interdisciplinary team meetings as appropriate.
Provide education and training to patients, families, and facility staff.
Complete timely electronic documentation, treatment notes, and daily billing activities.
Assist with discharge planning and continuity of care.
Support restorative and functional maintenance programs within the facility.
Maintain productivity and documentation standards while delivering high-quality patient-centered care.
