About The Position

Licensed Home Care Physical Therapist Assistant (PTA) - Enjoy independence and flexibility in the field to care for patients in their home environment. This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the southwest metro. Interim HealthCare of the Twin Cities in Roseville, MN offers a wide variety of home care ranging from skilled nursing, physical therapy and other licensed health care services, which help with the every day tasks that let people continue living at home where they are most comfortable. Additionally, we provide hospice care in the home that can make loved ones as comfortable as possible and provide support to the family, allowing them to concentrate on spending quality time together. Requirements

  • Currently licensed as a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
  • Two (2) years of experience as a PTA.
  • Able to travel within geographic area serviced by the office.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
  • CPR/BLS Certification

Responsibilities

  • Provide services for treating impairments, functional limitations, disabilities or changes in physical function under the direction and supervision of a qualified physical therapist.
  • Participates in ongoing interdisciplinary assessment of the patient.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Completes and submits timely, legible, accurate, relevant and sufficient documentation addressing the treatment and interventions provided, the patient’s response to the treatment and interventions, and the patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and payer requirements.
  • Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
  • Get the opportunity to see patients in their home environment to get a true representation of each patients' needs.
