About The Position

Covenant Home Health Agency (CHH) is seeking a Physical Therapist Assistant to provide skilled physical therapy services to patients across Senior Living Communities. The role supports seniors in recovery, improving mobility, and maintaining independence through tailored physical therapy treatment plans. The team collaborates to promote independence, enhance quality of life, and help residents reach their fullest potential. The position is PRN and covers the West Valley and East Valley.

Requirements

• Graduate of an accredited physical therapy assistant program with an Associate’s degree in science or applied science approved by the American Physical Therapy Association, Inc.
• Current certification or license as required by state law.
• Good verbal and written communication and organizational skills.
• Maintain current CPR certification.
• Licensed driver with an automobile in good working order and insured according to the organization’s requirements.
• Able to withstand prolonged or considerable walking, standing, reaching, stooping, bending, kneeling, or crouching.
• Able to lift, position, or transfer patients, supplies, and equipment; minimum lifting requirement is 25 pounds.
• Visual acuity and hearing sufficient to perform required therapy skills.
• Able to complete a max-assist patient transfer independently.
