About The Position
Relient Health is seeking a caring and compassionate Physical Therapist Assistant (PTA) to work in a Skilled Nursing Facility (SNF) setting in San Jose, CA. This is a full-time, direct hire/permanent placement position in a respected SNF known for high-quality rehab services and a collaborative, patient-focused therapy department. The role offers a supportive rehab team, modern equipment, mentorship, and a culture that values professional growth. New graduates are welcome.
Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Current CA license (or eligibility to obtain)
- New grads and experienced PTAs welcome!
- Strong communication, documentation, and interpersonal skills
Responsibilities
- Provide therapy services under the direction of a licensed Physical Therapist
- Assist in developing and implementing individualized treatment plans
- Document patient progress using EMR systems
- Foster strong communication with PTs, OTs, SLPs, and nursing staff
- Promote patient independence and mobility
