Provides physical therapy services to patients as prescribed by the attending physician under the direct (but not on-site) supervision of a licensed Physical Therapist. Treats patients to relieve pain and/or develop or restore function to reach the highest level of independence possible. Instructs patients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.). Writes clinical progress notes on the date of the visit for inclusion into the patient’s permanent record. Attends Physical Therapy staff meetings, conducts in-service presentations and case conferences. Performs an on-site, joint visit with the assigned Physical Therapist initially, and communicates regularly for instruction in changes in the patient’s program. Communicates regularly with the case manager and other health care professionals assigned to a case. Instructs patients and their families in transfer techniques, home exercise program and activities of daily living as assigned by the physical therapist. Other duties as assigned.
