About The Position
Physical therapy in the home is different. You’re not working through a list of patients in a clinic. You’re helping people regain strength, mobility, and confidence in the place they live every day. At Riverside Home Health & Hospice, our PTAs play a critical role in helping patients recover, avoid hospitalizations, and return to the activities that matter most to them. If you’re a PTA who values independence, patient connection, and meaningful outcomes — this is where your work makes a real difference. As a PTA in home health, your work directly impacts patient recovery and safety. You will help patients regain strength, balance, and mobility, support recovery after illness, surgery, or hospitalization, reduce fall risk and prevent re-hospitalizations, carry out therapy plans developed by the Physical Therapist, and help patients remain safely in their homes. Your work helps patients move from “I can’t” to “I can again.”
Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Licensed PTA in the State of Oregon
- Strong communication and organizational skills
- Ability to work independently in the field
- Valid driver’s license and reliable transportation
Nice To Haves
- Home health experience preferred but not required.
- Home health experience is helpful, but many PTAs find this becomes their preferred way to practice.
Responsibilities
- Providing physical therapy treatments in patients’ homes
- Following care plans established by the Physical Therapist
- Monitoring patient progress and reporting changes
- Educating patients and caregivers
- Documenting visits and collaborating with the care team
- Help patients regain strength, balance, and mobility
- Support recovery after illness, surgery, or hospitalization
- Reduce fall risk and prevent re-hospitalizations
- Carry out therapy plans developed by the Physical Therapist
- Help patients remain safely in their homes
