About The Position

Brookestone Home Health Care is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our team serving Elkhorn and the surrounding communities. As a PTA, you will work directly with patients in their homes, helping them improve mobility, regain independence, and achieve their rehabilitation goals under the direction of a licensed Physical Therapist. Requirements

Graduate of an accredited Physical Therapist Assistant program.
Current Nebraska PTA license in good standing.
Current CPR certification.
Valid driver's license, dependable transportation, and proof of automobile insurance.
Strong communication and organizational skills.
Ability to work independently while maintaining close collaboration with the supervising therapist.

Nice To Haves

Home health experience preferred, but not required.

Responsibilities

Administer physical therapy treatments according to the established plan of care.
Observe and report patient progress, responses, and concerns to the supervising Physical Therapist.
Educate patients and caregivers on exercise programs, mobility techniques, and home safety.
Maintain accurate, timely, and compliant clinical documentation.
Participate in care coordination with physicians, nurses, therapists, and other healthcare professionals.
Promote patient independence, safety, and quality of life.
Travel to patient homes within the assigned service area.
