About The Position

TheraCare is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout San Antonio and surrounding areas. This full-time home health PTA position is ideal for clinicians looking for flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting. Therapists enjoy creating their own schedules while helping patients improve mobility, strength, safety, and overall independence in the comfort of their homes.

Requirements

  • Associate degree from an accredited Physical Therapist Assistant (PTA) program
  • Active Physical Therapist Assistant (PTA) license in the state of Texas
  • Home Health experience (1 year required)
  • Current CPR certification
  • Strong communication, clinical judgment, and time-management skills
  • Valid driver’s license, reliable transportation, and current auto insurance

  • HomeCareHomeBase (HCHB) experience (preferred)

Responsibilities

  • Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT)
  • Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT
  • Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions
  • Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities
  • Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments
  • Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies
  • Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist
  • Complete accurate and timely electronic documentation in compliance with Medicare home health regulations
  • Follow infection control standards and home health safety protocols
  • Participate in QAPI and agency performance improvement initiatives
