About The Position

We are seeking compassionate, reliable, and skilled Physical Therapist Assistants (PTAs) to join our home health team. You will work under the direction and supervision of a licensed Physical Therapist to implement personalized treatment plans that help patients recover from injury, surgery, or illness and regain independence in their daily lives.

Requirements

  • Graduate of an accredited Physical Therapist Assistant program
  • Current PTA license in [State] (or eligible for immediate licensure)
  • Valid driver’s license, reliable transportation, and auto insurance
  • Current CPR/BLS certification
  • Excellent time-management and documentation skills
  • Compassionate, patient-centered approach with the ability to work independently

Nice To Haves

  • Minimum 1 year of clinical experience preferred (new grads with strong home-health interest will be considered)
  • Home health experience is a plus but not required – we provide training and mentorship

Responsibilities

  • Provide skilled physical therapy interventions in patients’ homes as outlined in the PT’s plan of care
  • Assist patients with therapeutic exercises, functional mobility training, gait training, transfer training, and use of assistive devices
  • Perform manual therapy techniques, modalities (ultrasound, e-stim, etc.), and therapeutic activities
  • Educate patients and caregivers on home exercise programs, safety, and fall prevention
  • Document progress notes accurately and timely in our EMR system (experience with Homecare Homebase or similar preferred)
  • Communicate effectively with the supervising PT, case managers, and other members of the interdisciplinary team
  • Manage a productive caseload while maintaining compliance with Medicare and state regulations
