About The Position

HealthPro Heritage has a great opportunity for a Licensed Physical Therapist Assistant (LPTA). This full-time position is located at Presidential Oaks in Concord, NH, within a Long-Term Care setting. The rate range is $33-$40/hr, based on years of experience. Presidential Oaks offers a diverse patient population including rehab, long-term care, and the aging population, allowing for a continuum of care where you can follow patient progress over time. You will work in a team environment with PT, OT, and nursing collaboration, and have the opportunity to build relationships with residents, providing more personal care. This role is ideal for gaining geriatric rehab experience, a high-demand skill, and offers a stable schedule, less hectic than hospital or outpatient settings. The environment is supportive and home-like, with strong job stability due to the growing senior population.

Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible.
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
