About The Position

Home Health Care, Inc. is seeking a Physical Therapist Assistant (PTA) to provide one-on-one Physical Therapy treatments in patients' homes within the Western Twin Cities Metro and Buffalo Area. This full-time role offers a per-visit compensation model with an annualized earning potential of $70,000–$102,000, plus mileage reimbursement. The position emphasizes independence, ownership, and the opportunity to build a strong career in home health by directly impacting earnings through productivity. The role involves managing a field-based schedule, collaborating with supervising Physical Therapists, and helping patients achieve goals related to mobility, independence, and safety.

Requirements

  • Active Minnesota Physical Therapist Assistant (PTA) license
  • Strong understanding of therapeutic exercise and functional mobility
  • Excellent patient and caregiver communication
  • Ability to independently organize and manage a field-based schedule
  • Strong collaboration with supervising Physical Therapists
  • Ability to recognize and communicate changes in patient condition
  • Dependable, motivated, and comfortable working independently
  • Valid driver's license and reliable transportation
  • Ability to travel throughout your assigned Western Metro & Buffalo-area territory

Responsibilities

  • Provide one-on-one Physical Therapy treatments in patients' homes
  • Carry out individualized plans of care established by the Physical Therapist
  • Provide therapeutic exercise and strengthening
  • Perform gait, balance, transfer, and mobility training
  • Help improve endurance, coordination, and functional independence
  • Reinforce home exercise programs
  • Educate patients and caregivers on safe mobility and therapeutic techniques
  • Monitor patient response to treatment
  • Recognize and communicate changes in condition
  • Provide progress updates to the supervising Physical Therapist
  • Collaborate with nursing, OT, SLP, physicians, and interdisciplinary clinicians
  • Complete timely and accurate clinical documentation
