About The Position

UPLIFT HOMECARE, LLC is seeking a compassionate and dependable Licensed Physical Therapist Assistant (PTA) to join our growing home health team serving Ruskin and surrounding communities. This position offers the opportunity to provide one-on-one patient care in the comfort of patients’ homes while working alongside a supportive clinical team dedicated to improving quality of life and promoting independence. UPLIFT HOMECARE, LLC provides compassionate, professional in-home care services tailored to seniors and veterans. Requirements

  • Current Florida Physical Therapist Assistant (PTA) license
  • Graduate of an accredited Physical Therapist Assistant program
  • Successful completion of the national PTA examination
  • Minimum of 1 year of home health experience required
  • Strong communication and documentation skills
  • Current CPR Certification
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to travel to patient homes throughout Riverview and surrounding areas

Responsibilities

  • Implement physical therapy services according to the treatment plan established by the supervising Physical Therapist and physician.
  • Provide therapeutic exercises, treatments, and interventions to relieve pain and restore function.
  • Observe, document, and report patient responses and condition changes to the supervising PT and Clinical Manager.
  • Assist in evaluating patient functional abilities and revising care plans as needed.
  • Complete clinical notes and progress reports accurately and within required timeframes.
  • Participate in care coordination conferences and interdisciplinary communication.
  • Educate patients and caregivers on exercises, mobility, safety, and treatment goals.
  • Participate in quality improvement initiatives and in-service education programs.
  • Maintain current knowledge of physical therapy practices and standards.
