About The Position

We’re seeking a compassionate and driven Physical Therapist Assistant (PTA) to join our therapy-owned home health agency. Our leadership understands what it means to be in the field—because we’ve been there. In this role, you’ll work closely with Physical Therapists to deliver personalized, high-quality care to patients in their homes, helping them regain mobility, reduce pain, and return to independence. This therapy-owned model ensures clinically focused leadership that understands your role, realistic caseloads and productivity expectations, strong support from experienced therapists, and a patient-first—not corporate-driven—care model.

Requirements

• Graduate of an accredited PTA program
• Current state PTA license (required)
• AHA CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

• Home health experience preferred
• HomeCare HomeBase experience is a plus

Responsibilities

• Carry out individualized treatment plans under the direction of a Physical Therapist
• Lead patients through therapeutic exercises and mobility training
• Apply modalities such as ultrasound and electrical stimulation
• Monitor progress and communicate updates to the supervising PT
• Educate patients and families on home exercises and safety strategies
• Maintain a safe and organized care environment
