About The Position

Physical Therapist Assistant (PTA) Help Residents Rediscover Independence, Strength, and Confidence — One Meaningful Milestone at a Time. At Mattapan Health & Rehabilitation Center, rehabilitation is about more than completing exercises or meeting therapy goals — it's about helping residents return to the activities, routines, and moments that matter most. We are seeking a compassionate and clinically driven Physical Therapist Assistant (PTA) to join our rehabilitation team. In this role, you'll provide skilled therapy services under the direction of a licensed Physical Therapist while building meaningful relationships with residents and helping them achieve their individualized goals. Our 85-bed skilled nursing and rehabilitation community provides both Short-Term Rehabilitation and Long-Term Care, giving you the opportunity to work with residents recovering from illness, injury, surgery, or functional decline, as well as those requiring ongoing maintenance therapy. As a valued member of our interdisciplinary team, you'll collaborate with Physical Therapists, nursing staff, and other healthcare professionals to support resident progress, promote independence, and improve quality of life through personalized care.

Requirements

Graduation from an accredited Physical Therapist Assistant program required.
Current state license or eligibility for licensure as a Physical Therapist Assistant required.
Current CPR/BLS certification required.
Strong interpersonal and communication skills.
Ability to work collaboratively with residents, families, and interdisciplinary healthcare teams.
Compassionate, dependable, and resident-focused approach to care.
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

Nice To Haves

Long-term care or skilled nursing experience preferred.
Experience working with geriatric and subacute rehabilitation populations preferred.

Responsibilities

Assist the Physical Therapist in planning, developing, and implementing individualized treatment programs for residents.
Provide skilled physical therapy treatments according to the established plan of care and within the scope of practice.
Assist residents in improving mobility, strength, balance, coordination, and functional abilities.
Monitor resident progress and document observations, treatments, and outcomes accurately and timely.
Maintain clinical documentation, treatment records, and departmental reports according to regulatory requirements.
Collaborate with interdisciplinary team members regarding resident progress, care planning, and discharge recommendations.
Educate residents, families, and caregivers on therapy techniques and recommendations to support continued progress.
Manage assigned caseload by coordinating individual and group treatment sessions.
Assist with maintaining therapy equipment and treatment areas in a safe, clean, and organized manner.
Participate in continuing education, in-service training, and departmental meetings.
Support therapy aides and students as assigned through education and guidance.
Maintain resident confidentiality, privacy, and dignity in all interactions.
Follow all safety, infection control, and regulatory standards.
Perform other duties as assigned.
